This Memorandum includes sample privacy documents to use as models in implementing Homeland Security Presidential Directive 12 (HSPD-12) at U.S. federal agencies. Included are sample Privacy Act systems of records notices, Privacy Act statements, and a privacy impact assessment developed by a working group of privacy experts.
