A data conversion plan is

[a] document [that] describes the approach to converting legacy data to a new format/database. The plan describes the tool(s) to be used, the types of automated and manual processes to be used, how data anomalies and errors are handled, and how converted data will be validated to ensure it is correct prior to its use in the new production system. This plan may be either a project office or contractor responsibility.[1]
