Bureaus of Vital Statistics maintain birth certificate records. Birth certificates can provide a child’s name, sex, date of birth, and address of place of birth; the names of the attending physician, midwife, and/or other assistants; the parents’ names, ages, addresses, race, places of birth, and occupations; the mother’s maiden name; and the number of siblings.
